Survey Team Member Checklist
Supervisory Cytotechnologists interested in serving as a Consultant Cytotechnologist on the ASCT Services, Inc. Survey Team, should submit the following information to Beverly Haigler-Daly:
Copy of ASCT membership card
Copy of Board of Registry Certification, including registry number
Curriculum Vitae
Three letters of recommendation. At least one must be from a member of ASCT
Certificates of training in cytology technologies
Documentation of your established workload limit set by your pathologist
Documentation of your enrollment in a proficiency testing program
